Request For Reprinted W-2:

Harmony Staffing is now accepting requests for reprinted W-2‘s. Associates who are requesting a reprinted W-2 for 2016 or any other year will need to print the form below, fill it out completely and make a copy of your picture ID. Please email the form and copy of your picture ID to the email below or bring the following items to our office: Copy of picture ID and Completed form. Once we receive the request, we will send to our payroll company.  Reprinted W-2’s will be mailed out 24- 48 hours of receiving request.

In order to access the check stub online website, you must have Internet Explorer 10 or higher. To change the compatibility in your browser while on the website, hit F12 and a new window will pop up. Click on the drop down box for “Browser Mode” and make sure that “Internet Explorer 10 compatibility view” is selected.

Harmony Staffing Services W-2 Policy

Call Off Procedure

Harmony Staffing Services is your direct employer. If you are running late or absent, it is your responsibility to call Harmony Staffing and leave your name, phone number, company assigned and reason for tardy or absence.  During business hours of 8:00am-5:00pm, please follow up with a Harmony Staffing Services employee.

Assignment Abandonment Policy

Any Harmony Staffing Services employee who walks off of an assignment before the end of the day or does not return to complete an assignment without prior 48 hour notification to Harmony Staffing Services will be deemed to have abandoned the assignment(voluntarily quit) and will be subject to disciplinary action, up to and including termination.

Violators of this policy understand that their pay rate per hour will be reduced to the current minimum wage rate per hour for the hours still due to be paid.

Safety in the Workplace

Harmony Staffing Services, its customers and employees share in the responsibility of maintaining a safe work environment. You must follow all safety procedures and wear required personal protective equipment as instructed. You are required to notify your Harmony Staffing Services office immediately if given an unsafe task or if you feel you have not been given necessary training.

Work Related Injury Policy

If a work related injury occurs while working on a job site, you must report it to your current supervisor at the job location IMMEDIATELY. Once you notify your supervisor, you must call the Harmony Staffing Services office to report the situation as well.

Policy on a Drug-Free Workplace

All employees are expected and required to report to work on time and in appropriate metal and physical condition for work. Employees who are under the influence of alcohol or drugs on the job will be terminated immediately. You may be required to submit to drug testing in order to be considered for assignment or continuation of assignment. Harmony Staffing Services conducts random drug screens.
